About The Position

The Inpatient Acute Care Cardiology Advanced Health Practitioner (AHP) is responsible for providing clinical, procedural, administrative, and educational services for patients requiring cardiac care in the inpatient setting. Under the supervision of the Chief of Cardiology, the Heart & Vascular Manager for Inpatient Cardiology Advanced Practice Providers, and the Director of Advanced Practice Nursing, the Cardiology AHP functions as an integral member of the multidisciplinary care team and provides both collaborative and independent patient care services. The Cardiology AHP is expected to maintain effective communication with patients, families, physicians, nursing staff, and ancillary services to support optimal patient outcomes and efficient care delivery. This position requires rotation between the Stanyan and Parnassus campuses, as operationally needed.

Requirements

Responsibilities

  • Managing acute care cardiology patients in the inpatient setting
  • Providing cardiology consultation services for acute care patients, as requested
  • Performing pre- and post-procedural patient examinations, assessments, diagnoses, and ongoing management of care
  • Developing and implementing treatment plans and ensuring continuity and coordination of care
  • Performing approved clinical procedures within scope of practice and credentialed privileges
  • Participating in the development of clinical practice protocols and quality improvement initiatives
  • Educating patients, families, nursing staff, and other healthcare professionals regarding cardiac conditions, treatment plans, and follow-up care
  • Collaborating with interns, residents, fellows, attending physicians, and interdisciplinary team members to deliver high-quality patient care
  • Facilitating diagnostic testing, therapeutic interventions, discharge planning, and transitions of care
  • Promoting patient access, care coordination, and continuity across the inpatient continuum
