Job Purpose This role supports a telehealth pilot program designed to expand patient access and enhance care delivery through innovative virtual services. The Nurse, Pilot engages patients, supports care coordination, and contributes to the development and refinement of scalable care models aligned with ROMTech’s Clinician guided at-home rehab approach. The role also supports program evaluation by providing feedback on workflows, engagement strategies, and patient outcomes.

Requirements

  • Clinical experience as an LPN or RN
  • Active LPN or RN license with compact privileges
  • Strong communication and patient engagement skills
  • Organizational skills with attention to detail and compliance
  • Proficiency with electronic health records (EHR) and healthcare technology platforms
  • Ability to work collaboratively in a multidisciplinary clinical environment
  • Must be able to communicate clearly and professionally in both verbal and written formats.
  • Must be able to engage in active listening and express ideas effectively in person, by phone, and via virtual meetings.

Nice To Haves

  • Experience with telehealth care delivery models
  • Familiarity with Chronic Care Management (CCM) workflows
  • Interest in healthcare innovation and pilot program development
  • Prior telehealth or care management experience
  • Additional certifications related to care management or telehealth

Responsibilities

  • Conduct telehealth outreach to patients, providing education, follow-up, and support aligned with care plans
  • Monitor patient progress and assist with care coordination activities across the clinical team
  • Collaborate with a multidisciplinary care team to support patient goals and outcomes
  • Accurately document patient interactions in compliance with applicable regulations and organizational standards
  • Contribute feedback on pilot workflows, patient engagement strategies, and care delivery improvements
  • Support Chronic Care Management (CCM) initiatives as applicable to enhance patient outcomes
  • Participate in pilot-related meetings, training sessions, or program evaluations
  • Support data collection or reporting efforts tied to pilot performance metrics
  • Performs other mutually agreed upon duties consistent with the applicable engagement or contract
