About The Position

Serves as the point of contact for referring physicians, patients/families and caregivers to provide resources and assistance with accessing clinical, supportive care and financial services offered and available in the St. Joseph Healthcare System and community. Coordinates activities of the multidisciplinary team to ensure coordination of services and continuity of care for the patient. Anticipates barriers to care and services and intervenes to ensure the continuity of efficient/timely care. Facilitates transitions of care to assure that patients have appropriate referrals and follow-up appointments.
