About The Position

As a Nurse Navigator, you become the steady, trusted voice for up to 250 patients each month. Your outreach helps patients better manage chronic conditions, understand preventive care, and stay engaged in their health goals. Over 75% of your day is spent speaking directly with patients, offering education, compassion, and clarity. This is not a job where you wait for the phone to ring—you drive the connection.

Requirements

Active LPN license
2+ years of hands-on patient care experience
A HIPAA-compliant home office with secure high-speed internet
Experience in patient care coordination or case management
Strong clinical judgment and a heart for patient engagement
The ability to work reliably Monday–Friday without shifting schedules

Nice To Haves

Care coordination or case management certification
Experience with diverse patient populations
Comfort with various EHR systems and healthcare software

Responsibilities

Make up to 15 outbound patient calls daily
Conduct assessments within your LPN scope to understand each patient’s unique needs
Create and collaborate on personalized care plans
Serve as the communication bridge between patients, families, and providers
Educate patients on their conditions, treatment options, and preventive care
Track progress and adjust care plans as needed
Document interactions and work within multiple healthcare systems and tools
