About The Position
The Nurse Navigator is responsible for enhancing communication amongst the patient/family, the interdisciplinary team, and all interdepartmental needs. This role assists in the development of strategies to reduce Length of Stay (LOS) and re-admissions. The Nurse Navigator is part of the cardiovascular team which monitors LOS and re-admission data, monitors the care process, and develops approaches to improve care efficiency and patient outcomes. Other related duties as assigned.
