About The Position

The Nurse Navigator for the FIND (Following INcidental Diseases) & Lung Screening Program at Loyola Medicine plays a critical role in coordinating care for patients with incidental findings identified through radiology imaging. This position supports an innovative, integrated care model that combines nurse navigation with advanced health information technology to ensure timely follow-up, improved clinical outcomes, and an enhanced patient experience. The Nurse Navigator is responsible for facilitating communication across the continuum of care, including patients, primary care providers, specialists, and hospital departments. By guiding patients through standardized care pathways, this role ensures efficient coordination, timely access to services, and adherence to evidence-based follow-up for incidental findings and lung screening results. Participates in multidisciplinary rounds and supports nursing staff in development of individualized, evidence-based outcome oriented, safe and timely practices

Requirements

  • Registered Nurse (RN) license in the state of practice.
  • Experience in nursing, preferably in oncology, pulmonary, or care coordination.
  • Strong understanding of healthcare systems and patient navigation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in electronic health record (EHR) systems.
  • Bachelor of Science in Nursing (BSN) preferred.

Nice To Haves

  • Experience with incidental findings and lung screening programs.
  • Certification in relevant nursing specialty (e.g., OCN, ONC).
  • Advanced practice degree (e.g., MSN, DNP).
