About The Position

In conjunction with the Cardiology Providers, the Heart and Vascular Institute, and Radiology partners, the Advanced Cardiac Imaging nurse navigator is responsible for developing and implementing evidence-based patient navigation strategies to improve advanced cardiac imaging throughput, patient experience, and support protocols that improve imaging quality. The navigator collaborates with all other disciplines to provide education, support, and consultation to enhance the quality of care delivered and facilitates optimum development of all services related Cardiac Imaging. Requirements

  • Current Connecticut License Registered Nurse.
  • Bachelors in health related or business field.
  • At least three years of experience in a clinical area (cardiology) or program management.
  • Ability to work effectively with providers and other departments.
  • Ability to effectively and efficiently coordinate, implement and monitor the patient’s care, and to communicate openly and empathetically with patients and their family.
  • Ability to work independently, as well as collaboratively with multidisciplinary team in all aspects of defining and implementing the navigation program.
  • Strong ability to apply the nursing process as appropriate within the context of the organization’s framework for professional practice and in accordance with guidelines established by the team.
  • Demonstrates use of clinical skills and expertise in diagnosis, treatment, and resource options based on individualized needs.

Nice To Haves

  • Master’s degree in health related or business field preferred.
  • Three years’ cardiology experience is preferred.
  • Nursing certification related to focus such as Cardiac Vascular (CV-BC), or Medical Surgical (MEDSURG-BC) preferred.

Responsibilities

  • Communicates with the multidisciplinary team, patients, and their families to assist with scheduling, imaging protocols, and follow up appointments for Cardiac Imaging.
  • Coordinates care for cardiac imaging patients through planning and implementation of clinical protocols/practice management guidelines, as well as serving as a resource for the clinical practice in both Radiology and Cardiology.
  • Collect data for analysis of quality and outcome management.
  • Collaborates with provider’s offices to improve cohesiveness between hospital and office settings.
  • Monitors clinical outcomes pertaining to system issues and quality of care delivery.
  • Develops quality filters, audits, and case reviews.
  • Acts as an internal resource for staff in Radiology and Cardiology, for Advanced Cardiac Imaging studies.
  • Other duties as assigned.
