About The Position

This role involves two key functions: Nurse Navigator and ED Registered Nurse. As a Nurse Navigator, you will act as a liaison between patients, families, and healthcare providers, facilitating treatment plans, appointment scheduling, and discharge education. You will identify barriers to healthcare and connect patients with necessary resources. You will also coordinate bed needs for boarding patients, review geriatric and medically compromised patients for expedited placement, and ensure high-quality customer service. As an ED Registered Nurse, you will promote and restore patient health by implementing the nursing process, collaborating with physicians and multidisciplinary teams, and providing support to patients and families. You will identify patient care requirements, establish a compassionate environment, and promote patient independence through education. You will ensure quality of care by adhering to therapeutic standards, resolve patient problems using multidisciplinary strategies, and maintain a safe and clean working environment. You will protect patients and employees by adhering to infection-control policies, document patient care services, and maintain continuity among nursing teams. You will also maintain patient confidence by protecting information, ensure equipment operation, maintain nursing supplies inventory, and maintain professional knowledge by attending educational workshops and participating in professional societies. You will contribute to team effort by accomplishing related results as needed.

Responsibilities

  • Acts as a liaison between the patient, their families, and internal and external providers.
  • Establishes a strong rapport with patients, families and other members of the health care team and community.
  • Assists with the implementation of a treatment plan for patients being discharged from the department.
  • Facilitates appointment scheduling, referral process and providing clear discharge education to the patient and family when appropriate.
  • Recognizes barriers to healthcare and connects patients to the resources, services, and system providers during discharge.
  • Communicates with family members (next of kin) of impending patient discharges to expedite transport back to a facility or home.
  • Facilitates external transport to assist with more timely transport back to facilities.
  • Collaborates with ED Ambassadors, Patient Liaisons and the multidisciplinary team on Patient and Family Communication and Experience.
  • Daily review and triage of all boarding patients in the department, coordinating bed needs by hours boarding and clinical needs.
  • Daily review of our geriatric population as well as our medically compromised patients in the emergency department for expedited placement.
  • Ensures the highest quality customer service for patients and providers.
  • Facilitates teamwork and multidisciplinary care for patients, staff, and facility.
  • Documents all work in the EMR.
  • Works to identify technology that will assist with patient flow and patient experience.
  • Conducts discharge phone calls as workload allows.
  • Facilitates transport Request for patients with available in-patient beds
  • Maintains data related to discharge times, trends and patient satisfaction related to discharge process.
  • Promotes and restores patient’s health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members.
  • Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Promotes patient’s independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; and answering questions.
  • Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; and following hospital and nursing division’s philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  • Resolves patient problems and needs by utilizing multidisciplinary team strategies.
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations; and calling for assistance from health care support personnel.
  • Protects patients and employees by adhering to infection-control policies and protocols; medication administration and storage procedures; and controlled substance regulations.
  • Documents patient care services by charting in patient and department records.
  • Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
  • Maintains patient confidence and protects operations by keeping information confidential.
  • Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer’s instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; and evaluating new equipment and techniques.
  • Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; and using equipment and supplies as needed to accomplish job results.
  • Maintains professional and technical knowledge by attending educational workshops and participating in professional societies.
  • Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods.
  • Contributes to team effort by accomplishing related results as needed.
