About The Position

Capital Health is seeking a Nurse Navigator for an Outpatient, Full-Time, Day shift position in Hematology Oncology Specialists in Pennington, NJ. This role involves performing nursing care and treatment modalities appropriate for individual patients, including outpatient electronic medical record intake, assisting LIPs with procedures, and triaging patients in crisis. The Nurse Navigator will follow up with patients regarding recent ER/Hospital admissions, use screening tools to track patient performance and clinical outcomes, and support Capital Health's core mission of quality care, compassion, dignity, and a comprehensive continuum of care. The position requires integrating cultural, clinical, ethnic, and age-specific factors into the patient's plan of care, providing emotional and informational support, and teaching patients and families. Responsibilities include assessing the need for patient education, planning and documenting patient education, and evaluating patient comprehension and compliance. The Nurse Navigator will also call patients to explain laboratory and test results under the direction of the LIP, explain preparations and procedures, obtain and record patient history, and review laboratory and test results daily, prioritizing them for LIPs. The role involves preparing patients, exam rooms, and medical records, obtaining specimens, and recognizing and reacting appropriately to changes in patient condition. Administering and documenting medications and injections accurately and safely, e-prescribing medications, and performing medication reconciliation are also key duties. The Nurse Navigator will complete competency checklists, perform Quality Assurance activities, and follow ANA Code for Nurses and Capital Health System Standards of Practice. Additional responsibilities include documenting refrigerator temperatures, potentially participating in research studies, triaging and documenting patient medical phone calls, assessing mental status, and scheduling according to protocol. The role requires the ability to assess substance abuse/misuse and withdrawal symptoms, screen patients for danger to self or others, attend training and collaboration sessions, and act as an active team member. Planning, reviewing, establishing, and implementing procedures for communication between physicians, LSCW, community resources, and patients, facilitating transitions between resources, and identifying financial or social barriers are also part of the role. Referring patients to abuse/neglect/domestic violence and alcohol and substance abuse resources as needed, and performing other duties as assigned are expected. Demonstrating willingness to assist co-workers during periods of heightened patient care activity is also a requirement.

Requirements

Associate's degree or graduate of accredited school of nursing.
NJ or PA license based on primary State office location required.
Must attain second out of state RN license (NJ or PA) within three months from date of hire.
Associate's degree requires eight years nursing experience in related specialty, inpatient and/or outpatient setting.
Bachelor's degree requires four years nursing experience in behavioral health, or inpatient and/or outpatient setting.

Responsibilities

Prepares patients, exam rooms and medical records for scheduled and non-scheduled patients.
Obtains specimens, identifies patients and specimens according to CH policy.
Recognizes important changes in patient condition and reacts appropriately in the Ambulatory setting according to CH policy.
Administers and documents medications accurately and safely according to established protocol.
Performs and demonstrates technical skills specific to assigned Ambulatory setting.
Administers injections as ordered by LIP according to established protocol and documents.
E-prescribes medications to pharmacies under direction of the LIP.
Performs Medication reconciliation.
Completes competency checklist with their preceptors.
Performs Quality Assurance activities as defined by Ambulatory setting.
Follows ANA Code for Nurses and Capital Health System Standards of Practice.
Documents refrigerator temperatures for proper storage of medications and specimens and takes appropriate action if outside acceptable range.
May participate in Research studies depending on practice.
Triages and documents patient medical phone calls, assessing patients' mental status, and schedules according to established protocol.
Ability to assess substance abuse/misuse and withdrawal symptoms; and ability to screen patient for danger to self or others.
