About The Position

The oncology nurse navigator is responsible for coordination of care across the continuum for hematology/oncology patients. This position is responsible for the overall coordination of care for the individual by facilitating timely appointments, communication between providers, etc. This position serves as a link to community services and works collaboratively with other healthcare team members to help achieve the goals of improved patient care, experience and outcomes. This position is full-time in Muskogee, but will travel to Tulsa for training and to attend meetings. Saint Francis is a Catholic, not-for-profit Health system based in Tulsa, Oklahoma. Requirements

  • 3 years related experience
  • Previous Oncology experience with a deep knowledge of cancer disease processes and treatment regimens
  • Passionate about patient education
  • Ability to build and maintain relationships with physician offices/referral sources
  • Compassionate with patients and their families
  • Strong patient advocate - consistently goes above and beyond to get the patient the care they need
  • Understand Oncology modalities and rationales
  • Understand current NCCN guidelines
  • Completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
  • Valid multi-state or State of Oklahoma Registered Nurse License.
  • 3 - 4 years related experience.
  • Customer-focused approach, with a broad definition of customer.
  • Proficiency in applying nursing theories, evidence-based nursing knowledge, problem solving skills.
  • Knowledge of pathophysiology of specific disease processes.
  • Understanding of oncology treatment modalities and rationales including surgery, chemotherapy, radiation therapy, symptom management, survivor care, and surveillance.
  • Understanding of current clinical guidelines for cancer treatment plan (i.e., NCCN guidelines).
  • Effective skills in collaboration, coaching, mentoring, listening, participative decision-making, negotiating and team building skills.
  • Effective verbal and written communication skills, tact and positive demeanor.
  • Ability and desire to establish and maintain rapport with physicians and organizational personnel.
  • Outcomes driven decision making.
  • Ability to plan, develop, implement and administer health related programs.
  • Ability to work autonomously, establishing benchmarks for success, and reporting these.
  • Demonstrates understanding of and commitment to the organization's mission and values.

Nice To Haves

  • A minimum of three years experience in an oncology nursing or patient education role preferred.

Responsibilities

  • Participates in multidisciplinary tumor boards/conferences and assists in facilitating the plan of care determined with the goal of helping to expedite the diagnosis and treatment process.
  • Establishes patient relationships by assessing the patient's understanding of plan of care, assisting patients and families with understanding medical terminology, adjusting to diagnosis and adherence to treatment plan.
  • Performs nursing functions of proficiently maintaining accurate documentation, using appropriate channels of referral and effectively communicating with all healthcare team members.
  • Provides continuity of care and remains as a point of contact throughout the entire cancer care experience from pre-diagnosis through survivorship, palliative/hospice care and end of life, as applicable.
  • Coordinates appointments and at times attending consultations as warranted, meeting with patients and families and being available by phone for patient questions and concerns.
  • Provides consistent reinforcement of education and understanding of disease process and treatment process to patients and families.
  • Contributes to efforts to assure quality services are provided in a timely and cost effective manner.
  • Actively participates in the Oncology Service Line and Cancer Committee.
  • Attends community health fairs and events as part of outreach activities to help educate the public about cancer prevention, etc.
  • Coordination of care across the continuum for hematology/oncology patients.
