About The Position

This position is responsible for providing education and support to metabolic and weight management patients and facilitating communication across the continuum of care. Responsibilities include management/navigation of patients, education and advocacy, and coordination of care with managing physicians, assurance of high-quality care, continuous process improvement, optimal productivity, efficient utilization of resources and customer satisfaction in the Metabolic programs for OhioHealth. Coordination of care will be over an extended period of time and involves multiple modalities of treatment screening referrals, patient communication, direct patient rooming functions, in person patient education, physician communication, following up on order placement and test completion, and a component of data tracking and registry work.

Requirements

  • Bachelor's Degree (Required)
  • BLS - Basic Life Support - American Heart Association
  • RN - Registered Nurse - Ohio Board of Nursing
  • Graduate of an accredited school of nursing and currently licensed in the State of Ohio.
  • Bachelor of science in Nursing from a four-year college or university.
  • Licensed to practice as Registered Nurse in Ohio; current Basic Life (BLS) provider
  • Clinical knowledge of surgery processes and weight management issues.
  • Able to establish supportive relationships with a diverse group of patients; exceptional customer service with specialized training in patient care
  • Experience initiatives; detail oriented with exceptional organizational skills
  • Experience as staff RN ( 5 -10 yrs. clinical, bedside nursing in related area of practice).

Responsibilities

  • Provides a customer focused and supportive environment for weight loss activities.
  • Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to surgical weight management program patients and families.
  • Coordination of care with managing physicians and multi-disciplinary team.
  • Establishes system for coordinating the patients care throughout continuum, spanning each level of care in which care is provided and coordinating interdisciplinary communication
  • Expands patient education throughout continuum and assists with documentation in patient record.
  • Initiates and contributes to modifications/changes in caregiver practice patterns defined by outcomes: improved continuity and quality of patient care; increased participation and satisfaction of patients and families; efficient allocation of resources, appropriate length of stay and improved communication between disciplines.
  • Routinely and systematically measure function and well being of patients and disease population-specific clinical and financial outcomes, at appropriate time intervals.
  • Addresses, plans and implements clinical process improvement initiatives through Multi-Disciplinary Teams.
  • Participates in clinical quality and process improvement activities
  • Identifies educational and marketing needs with multidisciplinary team throughout continuum of care.
  • Collaborates with leadership triad Physician Medical Director, Nurse Manager to assure successful implementation of projects.
  • Acts as leader, mentor to staff and other colleagues in connection with initiatives associated with related patient population.
  • Remains current on new/evolving technology, healthcare issues and standards.
  • Plans and organizes work effectively.
  • Meets expectations for internal and external outcomes.
  • Fosters performance/growth, promotes participation management.
  • Develops/mentors others.
