The Nurse Navigator I is pivotal in enhancing Navi Nurses’ operational effectiveness by bringing a blend of warmth and professional credibility to the initial client engagement process. This role demonstrates profound clinical expertise in aiding both new and established clients to devise personalized care plans. With the collaborative support of our administrative team and fellow nurse contractors, the Nurse Navigator I ensures unparalleled service quality in every client interaction and care plan creation. Focused particularly on our subacute client segment, the Nurse Navigator I is instrumental in managing various aspects of client care. This includes overseeing client management, conducting clinical assessments, advocating for client needs, supporting nursing staff, guiding the development of care plans, reporting on data, and handling general clinical operations. For clients with subacute care needs, this role involves crafting initial care plans via telehealth or phone consultations, managing ongoing care requirements, and addressing clinical questions from nurses. Critical needs are escalated to the Nurse Navigator II and Nursing Manager as necessary.

Requirements

  • Registered Nurse (RN) with an Arizona license.
  • Current AHA BLS and/or ACLS.
  • ASN or higher & graduated from an accredited nursing program.
  • 4+ years of direct client care experience, with at least 1 year in an acute setting.
  • Excellent interpersonal and communication skills.
  • Proficient with Google Suite, healthcare platforms, and adaptable to new technologies.
  • Complete the 20+ hour in-person training with DoN.
  • Enjoy and thrive in a fast-paced, startup environment with high levels of autonomy.
  • Must have access to a computer/laptop, phone, and reliable internet.

Responsibilities

  • Coordinate and manage client care plans for subacute needs, ensuring the highest quality of service.
