About The Position

The Nurse Navigator is an integral member of the Bone & Joint Institute multidisciplinary team, working collaboratively with physicians and advanced practice providers to coordinate care across the orthopedic continuum. The Nurse Navigator provides patient education, clinical support, and care coordination; facilitates communication between patients, families, providers, and support services; identifies and addresses barriers to care; and promotes efficient care delivery.

Requirements

• Minimum 3-5 years of nursing experience required.
• Demonstrated expertise in patient education, care coordination, navigation, multidisciplinary collaboration, and clinical triage.
• Proficiency with EHR documentation.
• BSN required.
• Current NJ-RN License or if working in an office in another state, that applicable state license.

Nice To Haves

• Orthopedic, musculoskeletal, surgical, or specialty practice experience preferred.
