About The Position
Responsible for coordinating the individualized care of patients throughout the continuum of care services, functioning as patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.
Requirements
- Five years’ experience as a nurse
- Two years supervisory/management and patient/staff education experience required
- RN License
