Nurse Navigator - General Surgical Clinic
About The Position
As the Nurse Navigator, you will coordinate and manage the care of general surgical patients, ensuring continuity of care delivered with compassion, excellence, and reliability. This role serves as a single point of contact for referring physicians, patients, and caregivers. You will operate in an expanded healthcare capacity, providing resources, education, and assistance to patients in accessing clinical and supportive care services available within the Munson Health Care system and surrounding communities. Key responsibilities include facilitating patient appointments for diagnostic services or clearances and serving as a vital resource for patients both before and after surgery. Additionally, you will support quality improvement initiatives and ensure adherence to safety standards.
Requirements
- Licensed RN in the State of Michigan
