About The Position

Acts as a patient advocate by providing education, support and interventions throughout the continuum of patient care including pre & post hospitalization. Performs duties by working with physicians to identify and carry out treatment plans. The Nurse Navigator - Congestive Heart Failure role is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

Requirements

• Bachelor's Degree - Nursing
• <2 years experience
• No Supervisor Experience
• RN License
• Ability to work in a constant state of alertness and in a safe manner

Nice To Haves

• Masters Degree
• Specialty certification, Certified Heart Failure Nurse or Cardiac Nurse Specialist
• Strong cardiology or ICU background
• OCN
• ONC

Responsibilities

• Conducts new patient interviews and assesses patient's function health and educational needs.
• Monitors patient's lab values, test results and progress.
• Provides patient/family education.
• Coordinates patient discharge needs to ensure smooth transition from hospital to outside agencies.
• Works with the Medical Director and other physicians to identify and carry out the treatment plan.
• Acts as a Patient Advocate helping to navigate patients through the diagnosis, treatment and rehabilitation process.
• Serves as a resource to assist patients and families, other professionals and the community.
• Provides education for the community and referring physicians.
• Facilitate clinical protocols established by Medical Director(s).
