About The Position

The Nurse Navigator, Advanced Lung Center is responsible for the coordination of testing and evaluation of all patients that present to the ALC for potential lung cancer screening and performance of the shared decision-making visit. This includes but is not limited to the performance of tobaccos cessation, lung cancer screening education, and coordination of follow up care. Works collaboratively with the Advanced Practice Provider (APP) assigned to the ALC to schedule and complete necessary follow-up on all completed testing. Acts as facilitator for the ALC Steering committee. Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Independence Health and the community. Participates in data analytics as it relates to utilization, quality, satisfaction, and outcomes. In conjunction with the providers for the ALC, facilitates the ALC steering committee, including data management and presentation and development of the agenda. The Nurse Navigator, ALC plays a critical role in guiding patients through complex thoracic and oncology care. This position requires strong clinical expertise, patient education skills, and experience coordinating care across multidisciplinary teams.

Requirements

  • Graduate of a Registered Nurse Program.
  • Minimum Two (2) to Three (3) years’ experience in Oncology, Critical Care, or Medical Surgical nursing experience.
  • Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
  • Must have excellent organizational skills and proficiency in Microsoft Office functions, Word, Excel, & PowerPoint.
  • Current licensure to practice as a Registered Nurse in the State of Pennsylvania required.
  • Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required.
  • Act 33 with renewal.
  • Act 34- PA Criminal Record Check of the PA State Policy System.
  • Act 73 FBI Clearance.

Nice To Haves

  • Bachelor of Science Degree in Nursing from an accredited college or university preferred.
  • Two (2) - Three (3) years of experience as a Nurse Navigator and/or patient educator within a service line.
  • Two (2) – Three (3) years of oncology nursing experience with the ability to proficiently review and interpret diagnostic imaging used in treatment planning.
  • Two (2) - Three (3) years of experience with ambulatory oncology case management.

Responsibilities

  • Maintain professional and technical knowledge by attending education workshops, review professional publications, establish personal networks, and participate in professional societies.

  • Document patient care services by charting patient and department records.
  • Maintain continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs using Nurse Knowledge Exchange techniques.
  • Maintain patient confidence and protect operations by keeping information confidential.
  • Identify patient care requirements by establishing personal rapport with potential and actual patients, and other persons in a position to understand care requirements.
  • Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Promote patient's independence by establishing patient care goals, teaching patient/family to understand condition, medications, and self-care skills, and answering questions.
  • Maintain safe and clean working environment by complying with procedures, rules, and regulations, calling for assistance from health care support personnel.
  • Resolve patient problems and needs by utilizing multidisciplinary team strategies.
  • Maintain a cooperative relationship among health care teams by communicating information, responding to requests, building rapport, and participating in team continuous quality improvement and problem-solving methods.
  • Contribute to team effort by accomplishing related results as needed.
  • Ensure operation of equipment by completing preventive maintenance requirements by following manufacturer's instructions, troubleshooting malfunctions, calling for repairs, maintaining equipment inventories, and evaluating new equipment and techniques.
  • Maintain nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies, placing, and expediting orders for supplies, verifying receipt of supplies, and using equipment and supplies as needed to accomplish job results.
  • Responsible for data collection, including but not limited to requirements for registry compliance, informational letters to patients, graphs for committee review, etc.
  • Other related duties as assigned.
