Canterbury Court is a retirement community located in the heart of Buckhead, where we put our hearts into creating a true community. As a faith-based, not-for-profit organization, we reinvest revenue into our community for the benefit of residents and staff. We are Atlanta’s first Life Plan Community (LPC) – what some call a Continuing Care Retirement Community (CCRC). Canterbury Court is looking for a Neighborhood Nurse Manager (Sage Glen) to join our family. This is a full-time position located in the Healthcare division of our Life Plan Community (LPC). The Neighborhood Nurse Manager is responsible for supporting high-quality person centered resident care in the Sage Glen neighborhood through leadership, infection prevention oversight, medical record auditing, staff education, and clinical accountability. This role manages Sage Glen’s infection prevention and control activities, monitors documentation accuracy and completion, supervises neighborhood operations, and collaborates with leadership to promote resident safety, quality care, and adherence to Canterbury Courts mission and value statement. The Neighborhood Nurse Manager must have a sincere desire to serve older adults in a quality continuing care setting, providing for dignity of all residents through a high standard of caring professional. Services must be provided in accordance with the American Nurses Association code of ethic along with adherence to Georgia Department of Community Health regulations. This position is responsible for fostering a culture that enables residents to maximize their individuality, independence, and dignity. This climate shall provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

Requirements

Graduate of accredited School of Nursing and must be licensed by the State of Georgia
Must be CPR certified
Demonstrated problem solving and conflict resolution skills in issues of staff to staff, staff to resident, staff and family, and resident to resident and between various departments.
Strong verbal and written communication skills; Computer literate: Word, Excel, Power Point, PCC. Must be able to speak, read and write the English language.
Thorough understanding of Federal and State rules, regulations, policies, and guidelines governing labor, employee issues related to their job.
Must have skills in communication, assessment and critical thinking. He or she should be able to work effectively with a variety of disciplines in an individual and team setting.
Should have knowledge of State and Federal Health Care Regulations, Laws, and procedures
Strong inter-personal skills; Able to relate well with administrator, directors, managers, staff, and larger community; Able to collaborate across disciplines to integrate and coordinate care management issues.
Must be able to work in a culturally diverse environment and consistently exhibit behaviors of equal opportunity and inclusivity.

Responsibilities

Coordinate and manage ancillary medical services for Sage Glen neighborhood (dental, podiatry, consultant nurse pharmacist).
Be familiar with Canterbury Court policies and procedures as part of Canterbury’s interdisciplinary team to assure compliance with Georgia Department of Community Health regulations.
Participate in and assist with reporting for quarterly QAPI meetings.
Understand and meet all government requirements Skilled Nursing for Personal Care residents’ documentation.
Manage admission and discharge process of all residents and ensure all regulatory requirements are met.
Oversee the accuracy and completion of the medical record, including admission documents, care plan creation and timely updates, nursing documentation, resident assessments, physician orders, treatment records, incident documentation, and care-related charting.
Collaborate with charge nurses to create and revise task and assignment sheets for staff.
Participate in monthly management rounding of the neighborhood to ensure the living environment meets Canterbury and regulatory standards.
Work with the interdisciplinary team and administration to promote and protect resident rights.
Work with residents, family members, responsible parties, and staff to provide support, information, for taking a more proactive role in self-advocacy to improve the quality of life/care for individual residents and those who live and work within the Personal Care and the community at large.
Educate staff regarding cultural diversity and each staff member’s importance when caring for residents.
Manage and support Canterbury Court’s infection prevention and control program in accordance with applicable federal, state, and local regulations, as well as organizational policies and accepted long-term care practices.
Conduct infection surveillance, track trends, monitor outbreaks, and communicate findings and recommendations to nursing leadership and interdisciplinary team members.
Assist with implementation and monitoring of infection prevention policies, including hand hygiene, transmission-based precautions, isolation practices, immunization tracking, and outbreak response measures.
Conduct and report on routine audits related to adherence to infection control standards.
Serve as a unit manager by coordinating clinical operations on the assigned unit, monitoring resident status, addressing care concerns, and promoting continuity and quality of care.
Manage new admission processes for both temporary and long term residents, including ensuring orders and documentation are uploaded to the chart, assessments are completed, and rooms are in good order.
Actively participate in weekly care plan meeting by having an in depth understanding of each resident’s clinical conditions and being prepared to answer resident and family questions within scope of practice.
Take responsibility of Charge Nurse’s duties, as needed and directed by DON/ADON.
Assist with grievance resolution of from patients and families in collaboration with Social Worker.
Ensures that medical supplies, medications, residents’ linen, laundry, and equipment are in good order and well-stocked.
Can recognize significant changes in the condition of residents and take necessary action.
Have working knowledge of all Residents under Nursing Care on Sage Glen
Supervise and support nursing staff to ensure residents receive care and services consistent with care plans, physician orders, organization policies, and accepted standards of practice.
Hold staff accountable for quality of resident care, timely completion of assigned duties, adherence to infection control practices, and accurate documentation.
Work collaboratively with nursing management to identify educational needs and provide training, coaching, and in-service programs related to infection prevention, documentation, resident care practices, and regulatory compliance.
Collaborate with nursing leadership team to ensure CPR certification and licenses/certifications are up to date for all healthcare staff.
Follow up with individual staff members to confirm compliance with yearly physical.
