About The Position

Maintains the standards of nursing care and practice of the Nursing Department on the STR unit. Responsibility is centralized in three major areas: STR patient care management, caregiver management and unit management. Has delegated authority to make decisions in these areas for the unit. Assumes the broad responsibility of translating the objectives, policies and procedures of effective action. Responsible for developing solutions for patient, family, staff, and overall unit situations which arise.

Requirements

  • Current New York State Registered Professional Nursing license.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulation and guidelines that pertain to long-term care.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.
  • In addition to providing excellent clinical care, must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures, etc. that are necessary for providing quality care.
  • Must have patience, tact, as well as the willingness to handle difficult patients and/or their families.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • At least 2 years of acute care experience along with long term care experience.
  • Previous leadership responsibilities.

Responsibilities

  • Maintains own current knowledge of nursing care requirements for all patients on the unit.
  • Initiates internal investigations for any patient care issue and communicates to nursing administration any unusual events.
  • Provides nursing care as necessary to meet the patient’s care plan.
  • Assesses the patient and family response to nursing home placement, nursing care and services.
  • Coordinates plans for total services to patients, including planning for optimal utilization of staff for the delivery of nursing services to meet patient care needs.
  • Maintains constant interventions and interaction with unit staff for the purpose of evaluating the care provided and the development of the staff's capability to render care in changing circumstances.
  • Works with medical staff to ensure patients are seen per regulations and as needs dictate.
  • Administers direct nursing care to patients as deemed appropriate to maintain own clinical competence and/or to assess particular patient requirements.
  • Works closely with own staff and peers in developing improved nursing care programs and nursing practices for the patients and, where appropriate, makes recommendations for change to the Director of STR or Assistant Director of Nursing.
  • Is a positive role model for communicating facility initiatives and supports facility policies.
  • In conjunction with the human resources department, may interview and provide input on individual personnel for the patient unit.
  • Monitors the activities of nursing personnel assigned to the unit to insure adherence to standards of the unit and the nursing department.
  • Evaluates each member of the unit staff and takes appropriate action if performance issues arise. Identifies problem, with guidance from Director of STR or Assistant Director of Nursing and human resources, takes corrective measures and recommends disciplinary action when indicated.
  • Plans for, directs and/or participates in regularly-scheduled meetings and conferences with staff.
  • Works closely with the Director of Short Ter Rehab and Assistant Director of Nursing, to maintain the staffing pattern as well as the short-term and daily scheduling time pattern for the patient unit.
  • Determines the utilization of unit staff.
  • Is the primary clinical nursing resource for own nursing staff and for other areas where their expertise may be required.
  • Monitors for any trends which may be a system issue and develops a plan of correction.
  • Using the Director of STR or Assistant Director of Nursing as a resource, delegates authority for carrying out the daily operational activities related to patient care, supplies, equipment and the unit's physical environment.
  • Monitors personnel, supplies and budget for the STR unit.
  • Maintains a safe environment for patients and staff - one conducive to positive health care and comfort in accordance with established policies and procedures.
  • Maintains close working relationship with other managers whose departments provide support services to the patient unit. Initiates appropriate communications and planning to ensure a consistently high level of service to the unit and to effectively meet any special needs of the patients.
  • Participates in the budget process of the nursing department. Makes recommendations to the Director or Assistant Director for new programs and/or program changes with supporting data justifying the need.
  • Establishes appropriate communication mechanism for providing information to the shift supervisors covering the unit on off-shifts.
  • Provides unit-based orientation to students and other unit-based non-nursing personnel regarding structure and operation of the unit.
  • Attends nursing department meetings and disseminates the appropriate information to staff.
  • Utilizes the Director of Short Term Rehab Assistant Director of Nursing for consultation, support and guidance in program planning for the unit, in self-development and in seeking appropriate resources.
  • Develops goals and objectives with the Director of Short Term Rehab for unit programs on an annual basis and submits written reports.
  • Identifies goals for professional self-development and seeks continuing education opportunities to attain these goals.
  • Assumes facility supervisor responsibilities as needed.
  • Responsible for overall patient and family satisfaction on unit.
  • Utilizes appropriate resources within the Jewish Home and within the nursing department to provide consultation, education, technical or informational services as needed to self, staff or patients.
  • Supports and participates in on-going educational and research programs of the nursing department and the JHR.
  • Assists the Director of Short Term Rehab or Assistant Director of Nursing with staffing patterns and personnel needs for the designated patient care unit and submits information to support these recommendations.
  • Assists new staff members in the adjustment process and facilitates the development of an appropriate orientation program for the new employee through the staff and organizational development department.
  • Contributes and represents the nursing department through committees.
  • All other job duties as assigned.
