NURSE, LPN Responsible for providing specialized nursing services to patients and caregivers at home or in alternative settings. Provides technical nursing and skilled interventions as ordered by the physician. Instructs, motivates, and assists patients / caregivers in learning and improving ability to live independently at home.

Requirements

Associate degree in nursing or equivalent
Current and unrestricted LPN clinical license
Two (2) years of clinical nursing experience is required
Valid and unrestricted driver’s license

Nice To Haves

Five (5) years of related HME experience is preferred

Responsibilities

Ensures physician orders are followed and included in patients plan of care
Development and revision of the plan of care based on physician orders
Ensures nursing orders are carried out in a professional manner satisfying professional ethics, patient rights and State and Federal codes and regulations
Continually monitoring patient outcomes and reporting changes to supervisors to prevent hospital readmission
Participates in effective communication amongst other clinicians providing care to the patient
Completes and submits all required documentation within established agency guidelines
Communicates the patient’s progress to supervisor and make changes as necessary under the physician’s direction
Covers after hours, weekend and holiday on call to meet the branches need
Maintains confidentiality of all company and patient related information. Protects patient rights as defined in the patient Bill of Rights
Attends and participates in staff meetings, required in-services and other meetings as requested
Instructs patients/caregivers of the safety measures appropriate to care and environment and observes/implements standard precautions and infection control for all patients.
Ensures accurate patient assessments are completed timely
Coordination of related services for patients
Response to emergency patient needs timely and escalating to supervisors and physicians as needed
Always represents self in a professional and ethical manner
Takes professional responsibility for self-protection during required work in potentially hazardous situations
Completes all required patient care documentation and reports accurately and in a timely manner
Communicates with clinical staff, physicians, and other health care providers to assure the quality and continuity of patient care
Other duties as assigned.
