About The Position
The person in this position will assist all program participants with any health care needs and/or issues related to health care treatment within the NuPath program. This person will assist in providing direct care services, as well as assisting and educating individuals to understand their health care needs to the best of their abilities.
Requirements
- Current LPN License
- One year experience working as a nurse in a community health setting.
- Experience using computers.
- Able to read, write and speak English.

Responsibilities
- Responsible for the medications distributed to program participants.
- Provide all emergency first aid to anyone in need.
- Relate, teach and further develop skills with the people served.
- Engage positively with members of the household, their families, and Team members.
- Respect and implement the NuPath Mission.
- Monitor and ensure the safety of individuals.
- Act in a polite, courteous and professional manner at all times.
- Adhere to all established NuPath procedures.
