About The Position
Our Lady of Mercy Life Center, part of St. Peter’s Health Partners, is seeking exceptional care team members for their Sub-Acute Rehabilitation Unit. This unit supports residents in regaining strength, independence, and confidence after hospitalization, providing compassionate, goal-oriented care in a warm, community-focused environment. They are hiring Per Diem RNs and LPNs with flexible commitment levels of 16 or 32 hours, offering all shifts (days, evenings, nights, weekends). These roles are ideal for nurses seeking flexibility to balance personal and professional priorities while making a meaningful impact with a dedicated, mission-driven team.
Requirements
- Current NYS RN license (for RN position)
- Associate Degree in Nursing or diploma from an accredited nursing program (for RN position)
- Current NYS LPN license (for LPN position)
Responsibilities
- Conducting comprehensive admission assessments
- Interpreting clinical and psychosocial data
- Developing and communicating individualized plans of care
- Educating residents and families on care and services
- Collaborating with an interdisciplinary team
- Administering medications and monitoring IV therapy
- Assisting with treatments and procedures
- Collecting resident data and contributing to care planning
- Providing emotional support to residents and families
- Ensuring safe, high-quality care in accordance with policies and standards
