About The Position
Our Lady of Mercy Life Center, part of St. Peter’s Health Partners, supports residents in regaining strength, independence, and confidence after hospitalization. The Sub-Acute Rehabilitation Unit provides compassionate, goal-oriented care in a warm, community-focused environment. We are hiring Per Diem RNs and LPNs with flexible commitment levels of 16 hours or 32 hours, offering all shifts, days, evening, nights, and weekends. These roles are ideal for nurses seeking flexibility while making a meaningful impact, allowing them to balance personal and professional priorities while working with a dedicated, mission-driven team.
Requirements
- Current NYS RN license
- Associate Degree in Nursing or diploma from an accredited nursing program
- Current NYS LPN license
Responsibilities
- Conducting comprehensive admission assessments
- Interpreting clinical and psychosocial data
- Developing and communicating individualized plans of care
- Educating residents and families on care and services
- Collaborating with an interdisciplinary team
- Administering medications and monitoring IV therapy
- Assisting with treatments and procedures
- Collecting resident data and contributing to care planning
- Providing emotional support to residents and families
- Ensuring safe, high-quality care in accordance with policies and standards
