About The Position

Roof Above is NC’s most comprehensive homeless service provider, with a mission to unite the community to end homelessness. Focusing on individuals experiencing homelessness, Roof Above offers a wide range of services from street outreach to permanent supportive housing. The program nurse, as part of a multi-disciplinary team, will address the physical and behavioral health needs of clients in a residential, non-clinical environment. This role involves collaborating with programming staff to assess needs, ensure safe and stable housing, and provide education on medications, diagnoses, and referrals. The nurse will work within residents' homes, single-site offices, and common spaces, adapting standard medical protocols to the PSH environment while adhering to scope of practice and agency policy. A key aspect of this role is building trust-based, therapeutic relationships with residents using trauma-informed, culturally responsive, and harm-reduction approaches, respecting their right to self-determination.

Requirements

A Bachelor’s degree
Must be a graduate from an accredited school of nursing.
Current licensure in the State of North Carolina as a Licensed Practical Nurse or a Registered Nurse.
Knowledge of, and commitment to, harm reduction and Housing First principles.
Strong written and oral communication skills.
Computer proficiency and familiarity with electronic documentation.
Superb collaborative skills and demonstrated success working in a high-needs environment.

Nice To Haves

Experience working with chronically homeless individuals strongly preferred.
Experience working with individuals with a history of mental illness and substance use preferred.

Responsibilities

Provide tenants with guidance and support around medical and behavioral health-related issues in a residential, non-clinical environment.
Reinforce education and counseling to clients regarding identified health needs, health care activities, chronic disease management, preventive care, medication effects/side effects, and contraindications.
Perform administrative and clinical support tasks, including documentation, coordinating appointments, obtaining vital signs, simple wound care, and supporting tenants in understanding how to self-administer their own pre-poured or prepackaged medications.
Exercise sound clinical judgment in situations where standard medical protocols may need to be adapted to the PSH environment.
Work closely with non-clinical staff and program staff (case managers, peer support specialists, property management) to align health interventions with housing goals.
Utilize trauma-informed, culturally responsive, and harm-reduction approaches when working with residents.
Build trust-based, therapeutic relationships with residents who may have experienced trauma, system involvement, and barriers to traditional healthcare.
Respect residents’ rights to self-determination, including the right to decline services or make choices that differ from medical recommendations.
Utilize creativity and flexibility to meet residents where they are.
Ensures confidentiality regarding sensitive material, including protected health information.
Coordinate care with primary care providers, behavioral health providers, hospitals, pharmacies, and other community health partners.
Become certified in Homeless Management and Information Services database to enter services and view records, as needed.
Perform other duties as assigned.
