About The Position

Responsible for identification and management of appropriate hospital and facility discharges, to include patients returning to the community or remaining in a facility setting. Provides backup for Intake Coordinator in the event of absence. Performs community education, liaison, and outreach activities with referral sources to ensure on-going awareness of agency’s services; works with local community agencies to generate a positive image and encourage word-of-mouth referrals. Responsible for relationship development and management of health partners and organizations; sources new referrals. Enhances relationship with area hospitals through regular contacts and participation in committees. Takes hospice referrals working collaboratively with discharge planners, including coordination with hospice clinical intake team; performs General Inpatient Assessments, Evaluations and admissions as needed. Assists in providing general information to patients and families; anticipates and eliminates potential conflicts between internal teams and health partners and/or families/residents. Submits reports on activities and accomplishments to the Administrator. Provides statistical analysis reports as needed. Development and management of data for the agency contacts and outreach efforts. Attends Team Meetings as needed; Available to attend AHS Ethics Committee; Newton Palliative Care Meetings. Appropriately seeks assistance when workload exceeds a reasonable level. Seeks opportunity to refine expertise in all areas of responsibility. Keeps abreast of technical and legal developments. Maintains relations with Atlantic Health Services and St.Luke’s through involvement in Palliative Care Services; serves as a resource to organizations on Hospice and Pain Management. Participates in planning and preparation of new endeavors, i.e. Palliative Care. Participates in establishing long and short-term goals for Hospice; Complete assignments in a timely, thorough and detailed manner. Appropriately seeks assistance when workload exceeds a reasonable level. Seeks opportunity to refine expertise in all areas of responsibility. Keeps abreast of technical and legal developments. Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations. Participates in all required in service education programs and meetings. Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance. Performs other duties as assigned

Requirements

  • Knowledge of the dying process
  • Knowledge of policies and procedures of the hospice organization
  • Knowledge of regulations surrounding compliance with Medicare Conditions of Participation
  • Ability to function as a resource to patients and families during times of high stress and volatility
  • Ability to assess patients to determine appropriateness of participation in hospice
  • Ability to build constructive relationships with colleagues, and outside agencies, organizations, and individuals
  • Excellent communication skills to effectively deliver information between referral source and the hospice clinical team
  • Consistently model respect for, and accountability to, the public, patients/families, and co-workers
  • Communication and actions reflect care, courtesy, honesty, and professionalism
  • Convey a professional and positive image and attitude regarding the hospice organization
  • Graduate of an NLN accredited school of nursing
  • Current licensure in New Jersey
  • RN required
  • 3-5 years working a healthcare environment with strong clinical skills
  • Exposure to hospice care
