About The Position

Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks a Per Diem Weekend Nurse Liaison to join its expanding weekend nurse team. This is an on-site position located at Newton Wellesley Hospital. Good Shepherd Community Care is one of the most trusted health care organizations in Greater Boston, focusing on hospice and palliative care. Founded as Hospice of the Good Shepherd in 1978, we are the original Massachusetts hospice. Requirements

  • Licensed by the Commonwealth of MA as a Registered Nurse.
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families
  • Highly skilled at providing patient assessments and evaluations.
  • Excellent problem-solving skills
  • Competency with accepted standards of hospice nursing practice.

Nice To Haves

  • At least two years experience preferred in community health, oncology, hospice, or palliative care.

Responsibilities

  • Provide information about hospice services to patients, families, and hospital-based staff.
  • Conduct clinical assessments.
  • Coordinate hospital discharge plans.
  • Maintain professional relationships within hospitals, with a focus on the assigned hospital(s).
