About The Position

Methodist Rehabilitation is seeking a PRN Nurse Liaison to serve as an important connection between patients, healthcare providers, referral sources, and the community. This role combines clinical knowledge, patient assessment, communication, relationship building, and rehabilitation education. The Nurse Liaison helps identify patients who may benefit from inpatient rehabilitation and works closely with healthcare professionals throughout the referral and admission process. Requirements

  • Graduate of an approved and accredited school of nursing.
  • Current Mississippi nursing license.
  • Strong clinical assessment and critical-thinking skills.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and relationship-building abilities.
  • Excellent computer and organizational skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Valid Mississippi driver's license, reliable transportation, and proof of automobile insurance required.
  • Ability and willingness to travel to various locations as needed.

Nice To Haves

  • Previous experience with pre-admission patient screenings preferred.
  • Experience working with physicians, referral sources, case managers, social workers, discharge planners, and/or insurance companies preferred.
  • Experience educating healthcare professionals about rehabilitation services preferred.

Responsibilities

  • Conduct pre-admission screenings to determine whether patients meet criteria for inpatient rehabilitation services.
  • Serve as a clinical liaison between MRC physicians, hospitals, healthcare providers, referral sources, patients, families, and the community.
  • Build and maintain positive relationships with physicians, case managers, social workers, discharge planners, insurance companies, and other referral partners.
  • Educate healthcare professionals and referral sources about MRC's rehabilitation services, patient population, and admission process.
  • Communicate effectively with referral sources and the interdisciplinary team to facilitate appropriate and timely admissions.
  • Gather and communicate relevant clinical information to support the admission and rehabilitation evaluation process.
  • Maintain accurate patient and referral information using electronic systems.
  • Provide excellent verbal and written communication with patients, families, healthcare professionals, and community partners.
  • Represent Methodist Rehabilitation Center professionally throughout the healthcare community.
  • Travel to hospitals, healthcare facilities, physician offices, and other locations as needed to conduct screenings and develop referral relationships.
  • Perform other duties related to the referral and admission process as assigned.
