Requirements
- Active Nebraska RN License (unencumbered)
- 2 years of skilled nurse experience (required)
- Knowledge of Medicare, Medicaid, managed care, and skilled nursing admission processes
- Strong critical thinking skills, independent judgment, and the ability to multitask and prioritize work assignments in a fast-paced work environment
- Ability to understand, read, write, and speak English

Responsibilities
- Conduct clinical consultations and guide admission decisions for prospective residents
- Coordinate and oversee incoming patient referrals and admissions
- Support census management by ensuring appropriate clinical placement across service lines based on patient needs
- Collaborate with hospital, clinicians, discharge planners, families, and referral partners to support continuity of care and timely admissions
- Review referrals for medical appropriateness and coordinate admissions
- Manage clinical intake, including records review, insurance verification, and clinical transitions
- Maintain positive relationships with referral sources and promote Maple Crest programs and services
