About The Position

This role involves supervising field staff and ensuring the delivery of quality care to clients. The Nurse Field Supervisor will assist field staff in meeting qualifications, provide education and guidance, monitor performance for regulatory adherence, and manage client admissions. The position requires performing supervisory visits to client homes, investigating incidents, and assisting with disciplinary actions. The supervisor is responsible for timely submission of records and reports, conducting performance evaluations, and adhering to all agency policies and procedures. Key attributes include dependability, honesty, independence, initiative, strong interpersonal skills, job knowledge, productivity, professional appearance, willingness to participate in special projects and training, and maintaining confidentiality. The role also emphasizes safety in the field and ensuring agency compliance with all relevant regulations.

Requirements

Must be eighteen years of age or older.
Must have completed an accredited LPN or RN nursing program and hold a current license to practice in Tennessee.
Must have a thorough knowledge of the principles and methods involved in supervising field staff (Personal Care Attendants and CNAs).
Must be capable of performing job duties physically, mentally, and independently.
Excellent communication and interpersonal skills.
Excellent organizational and time management skills.
Ability to work well independently and in groups.
Must have reliable transportation. The work requires the ability to safely transport oneself to and from multiple client homes and work sites on a regular, daily basis in a multi-county area.
The work will require some lifting, pulling, bending, pushing, stooping, kneeling, reaching, and carrying of light and heavy items, such as books, papers, files and/or office supplies, often at least 20 lbs. weight.
There will be short to long periods of transferring, standing, and sitting.
Must have mental ability to exercise good judgment in decision-making, including while driving and while in client homes.
Ability to walk independently up and down a normal flight of 13-15 stairs.

Nice To Haves

A thorough knowledge of the framework, organization, and function of the Personal Support Services program strongly preferred. Training may be possible for the right candidate.

Responsibilities

Assisting field staff in meeting the qualifications to deliver quality care to the clients.
Assisting in staff education including orientation, proficiency check-offs, and annual education programs as assigned by the upper management.
Supervising and exhibiting leadership capabilities and providing guidance and instruction to personal support staff as needed.
Monitoring field staff performance to assure adherence to insurance and state regulations.
Performing client admissions timely and accurately, as required. Solicits necessary client specific information to support the client’s admission to the agency as set forth by PSSA regulations.
Performing supervisory visits to client’s homes in a timely manner and submits related documentation timely. Visits will not exceed once per month nor be less than once per quarter.
Using initiative and self-direction to schedule admissions and supervisory visits the day prior.
Reporting negative comments, complaints, concerns/issues discovered during home visits, to upper management in a timely manner.
Assisting in the investigation of critical incidents as directed by upper management and documenting findings in a timely manner.
Assisting in providing disciplinary action to field employees as needed and directed from upper management.
Submitting records and reports timely as required by administration. Submits paperwork that is neat, legible, and written in dark colored ink or electronically, as required.
Performing field staff performance/competency evaluations within 30 days of employment and at least yearly thereafter.
Acknowledging and following policies and procedures, as outlined in the Employee Handbook.
Recording time worked accurately and using the approved method.
Willingness to travel to client homes.
Availability: Always on time and follows rules concerning attendance. Provides advance notice of absences or tardiness. Does not abuse time off practices.
Adherence to Policy: Always follows rules, procedures, and regulations. Completes and submits all paperwork on time.

Confidentiality: Maintains confidentiality of information related to business practices, business activities, client information and personnel information, including adherence to HIPAA and other privacy standards.
Safety: Safely utilizes cell phones and/or other communication devices while fulfilling job duties in the field, for business purposes only, while adhering to federal and state traffic and transportation regulations.
Ensures agency compliance with Federal, State, and Local regulations related to the delivery of Personal Support Services and makes available reports as needed.
Compliance: Understands and complies with laws, regulations, rules and policies. If unsure an action is permitted, seeks advice from supervisor. Accepts personal responsibility for preventing violations and speaking up if possible violations are identified.
