The Nurse Coordinator is a Registered Nurse (RN) responsible for delivering in-home patient care, conducting required supervisory visits, and providing field-based clinical support. This role operates in a part-time capacity and supports care delivery, assists with coordination efforts, and helps ensure continuity of care as needed. The Nurse Coordinator collaborates with Case Managers, physicians, and the interdisciplinary care team to support patient care in the field.
