Responsible for providing care coordination, care plan development, and skilled nursing assessment of patients referred to the Supportive Medicine consult service. Additionally anticipates and assists with symptom management and pain control; participates in family support, teaching, and discharge planning and continuity of care in collaboration with the primary care team. Participates in both formal and informal education for all disciplines.
