The Nurse Coordinator provides health care oversight and medical case management for an identified caseload of youth/individuals. This role may also include performing all clinical functions of a Registered Nurse (RN). We are seeking a dynamic, self-driven, and adaptable Registered Nurse with OMH experience (preferred) to join our team as a Nurse Coordinator for our newly launched Critical Time Transition Program (CTTP) – Next Steps. In this role, you will provide essential oversight and support to youth facing mental health challenges, advocate on their behalf with healthcare providers and other stakeholders, and supervise direct care staff. Full-time (40 hours per week) Monday – Friday, 8:30 AM – 5:00 PM (Flexibility to participate in on-call rotation required).
