About The Position

The Nurse Clinician utilizes coordinated treatment to provide comprehensive wound assessments and plan of care including goals in implementation of pressure injury prevention, treatment, evaluation and outcomes for patients throughout the organization. Provide patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Acts as a resource for physicians wound team and clinical staff. This role will include performing general nursing duties in all departments with adequate supervision. Participates in performance improvement plan of the organization.

Requirements

Licensed as a Registered Professional Nurse in the State of Illinois, is required (Must provide proof at the time of interview)
Bachelor of Science in Nursing (BSN) from a school of nursing, is required (Must provide official transcripts at time of interview)
Three (3) years of nursing experience in a hospital as a Staff Nurse, is required
Two (2) years of wound care experience, is required
Must possess a current Advanced Cardiac Life Support (ACLS) certification, is required (Must provide proof at the time of interview)
Must possess a current Basic Life Support (BLS) certification, is required (Must provide proof at the time of interview)

Nice To Haves

Wound, Ostomy and Continence Nursing certification (WOCN) or Wound Care Certification (WCC), is preferred

Responsibilities

Performs wound assessments, reassessments and wound management for all patients and per policy.
Provides wound care expertise, makes treatment plan recommendations, and provides status reports on healing wounds to physicians or primary health care providers.
Makes recommendations to the physicians for changes to wound care orders and provides evidence-based practice support as needed, i.e. bed surfaces.
Assist in the development of protocols and practice standards for managing patient with wound ostomy and incontinence care needs based on the latest and strongest evidence.
Monitors patient outcomes on an ongoing basis.
Works with the Nursing Department and staff on the development of education programs specific to Wound Ostomy Continence (WOC) patient care.
Provides basic wound care training to nursing staff, i.e., skills fairs, orientation, etc.
Utilizes a multi-disciplinary approach, collaborating with other members of the health care team to develop individualized plans of care for patients, i.e., dietary consults.
Participates in multi-disciplinary patient centered case conferences, as appropriate.
Maintains data and statistical reports as requested.
Participates in research projects as related to WOC issues.
Participates in performance improvement, i.e., prevalence studies for pressure injuries.
Assumes on-call responsibilities and acts as a resource person to nursing staff.
Uses a customer focused approach when collaborating with agency customers including workers, patients, physicians and staff from community agencies and institutions.
Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service education programs.
Demonstrates the knowledge and skill necessary to provide care appropriate to the age of the patients served on his/her assigned visit.
Supports the Corporate Compliance Program by adhering to the policies, procedures by adhering to the policies, procedures & department manuals of the organization.
Attends all mandatory in-services and annual training programs relating to Corporate Compliance.
Assures that activities the employee is responsible for comply with any federal and state regulations.
Reports and corrects any such conflicts which may arise.
Performs other duties as assigned.
