About The Position

Vancouver Coastal Health (VCH) is implementing minimum Nurse to Patient Ratios (mNPRs) to enhance nursing practice and quality of care. This initiative is a collaboration between the Ministry of Health, the BC Nurses’ Union (BCNU), and health organizations. This role as a Nurse Clinician/CRN is based at Three Bridges Community Health Centre in Vancouver, BC. Within a comprehensive care coordination and client/family-centred care model, the position applies specialized theory, clinical expertise, and knowledge to promote safe, ethical, and quality client care across multiple sites/clinics/services. Responsibilities include conducting one-on-one and group education/facilitation/counseling for staff and/or patients/clients/families, promoting excellence in client-centred care, education, and research, and participating in direct client and family care when necessary. Practice adheres to the British Columbia College of Nurses and Midwives (BCCNM) standards, organizational policies, and the client and family-centred care model.

The team supports Vancouver Community Adult Day Programs, Overnight Respite, and Meal Services, collaborating with service providers to help complex clients remain at home while offering respite for caregivers. The role involves collaboration with stakeholders in Vancouver, Richmond, and Coastal regions to enhance service delivery, quality of care, and client/caregiver experience.

Home Health programs offer a range of health care and support services to community members with acute, chronic, palliative, or rehabilitative needs. The client population typically has ongoing health issues and/or complex personal/social issues requiring care planning and resource coordination. Clients include frail elderly individuals, younger clients with disabilities, and adults with chronic illnesses needing support for independence. Three Bridges serves the Downtown, West End, Coal Harbour, Yaletown, and Fairview Slopes areas.

Requirements

• Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required.
• Completion of post basic specialty nursing certificate/program and three (3) years' recent, related experience (Home Health or Palliative Care) that includes experience developing and delivering education programs or an equivalent combination of education, training and experience.
• Valid BC Driver's License and access to personal vehicle for local area travel may be required.
• Demonstrated ability to provide clinical leadership/support.
• Knowledge of adult learning principles.
• Demonstrated ability to develop and deliver educational programs and materials for staff/patients/clients and families based on education theories, principles and best practices.
• Demonstrated ability to teach, demonstrate, coach and facilitate (one on one and in groups).
• Demonstrated ability to work independently and in collaboration with others.
• Knowledge of relevant clinical practice standards/guidelines and care pathways.
• Knowledge of other health care disciplines and their role in patient/client care.
• Demonstrated ability to effectively plan, organize and prioritize work.
• Demonstrated ability to effectively communicate in writing and orally on a one to one and group setting.

• Demonstrated computer literacy to operate a computerized client care information system as well as applicable word processing, spreadsheet and database software applications.
• Ability to operate related equipment.
• Demonstrated physical ability to perform the duties of the position.

Responsibilities

• Applies specialized theory, clinical expertise and knowledge relating to designated/specific client populations to promote the delivery of safe, ethical and quality client care.
• Conducts one-on-one and group education/facilitation/counseling sessions to staff and/or patients/clients/families as needed.
• Promotes excellence in client centred care, education and research.
• Participates in direct client and family care when required.
• Practices within the context of a client and family centred care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for registered nurses or registered psychiatric nurses.
• Practices within the scope of practice and code of ethics as set by the organization as per VCH policies and guidelines/clinical practice documents and within own competence.
• Collaborates with stakeholders in Vancouver, Richmond, and Coastal to improve service delivery, quality of care, as well as client and caregiver experience.
