About The Position

Rogue Community Health is seeking an RN Nurse Case Manager for their School Based Health Center locations. This role focuses on ensuring the efficient flow of students/patients during scheduled school hours and providing patient care and education within the RN scope of practice. The care will emphasize patient engagement, behavioral change, treatment compliance, and health risk factor reduction. School-Based Health Centers offer comprehensive physical, mental, and preventive health care services to children and adolescents within a school setting. Registered students and school staff are eligible for appointments with proper consent. The ideal candidate is committed to serving the patient population to facilitate empowerment and self-care.

Requirements

• Graduation from an accredited school of nursing.
• Must work 20 hours per week or more to be benefit-eligible.
• Must complete a criminal background check.
• Must complete a pre-employment drug screen once a conditional offer is accepted.
