About The Position
CarepathRx supports health systems and hospitals with pharmacy and infusion services designed to improve patient care. We are seeking a Case Management Analyst (RN) to coordinate admissions and ongoing care for patients receiving infusion therapy. You will work closely with patients, providers, pharmacists, home health agencies, and internal teams.
Requirements
- Graduate of an accredited nursing program.
- Active Registered Nurse (RN) license in PA or Compact.
- Strong communication, organization, and time management skills.
- Proficiency with Microsoft Office.
- Ability to manage multiple priorities in a fast-paced environment.
- Internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.
Nice To Haves
- Bachelor of Science in Nursing (BSN).
- Experience in home infusion, specialty pharmacy, case management, or home health.
Responsibilities
- Coordinate patient referrals and admissions.
- Confirm patients meet admission and clinical requirements.
- Communicate with patients, providers, pharmacists, home health agencies, and internal teams.
- Provide ongoing case management, including help with infusion pumps, supplies, and care transitions.
- Educate patients and caregivers about therapy needs and available resources.
- Deliver responsive customer service and help improve team processes.
