About The Position
Alignment Health is seeking a Remote Bilingual Spanish Nurse Case Manager – Care Transitions (RN, CA License) to join their team. This role focuses on serving seniors and the chronically ill, aiming to transform lives by providing exceptional care. The Nurse Case Manager will help members transition safely from hospital or skilled nursing facility stays back to their homes, providing education, support, and coordination that directly impacts health outcomes. This is a remote position with flexible scheduling, offering an opportunity to lead impactful patient care initiatives and collaborate with a supportive, interdisciplinary team.
Requirements
- Active, unrestricted RN license in California (willing to obtain other state licenses).
- Knowledge of Medicare Managed Care Plans, insurance regulations, and community resources.
- Strong communication, problem-solving, and organizational skills.
- Bilingual Spanish proficiency.
Nice To Haves
- 2–3 years of clinical care management experience (3–5 preferred).
Responsibilities
- Create individualized discharge plans with patients, families, and the care team.
- Coordinate care with providers, rehab facilities, and home health agencies.
- Monitor patient progress and adjust plans as needed.
- Educate and empower patients and families for self-care and follow-up.
- Advocate for patient needs and connect them to community support services.
- Participate in care conferences, quality improvement initiatives, and interdisciplinary collaboration.
