About The Position

Paradigm is seeking a Field Nurse Case Manager to support injured workers in the Elizabeth, NJ area and surrounding communities. This is a hybrid position requiring 80% local travel for in-person patient appointments. The role involves managing the medical care of work-related injuries, including assessment, planning, coordination, implementation, and evaluation. The Case Manager collaborates with insurance carriers, medical providers, attorneys, employers, and employees, monitoring injured worker progress and reporting results. The position requires willingness to learn and manage catastrophic cases. Requirements

  • Current, unencumbered compact Registered Nurse (RN) license in NJ.
  • Current, unencumbered Registered Nurse license in NY, required within 60 days of hire date.
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal).
  • Ability to operate autonomously with minimal oversight.
  • Skilled at patient education.
  • Valid Driver’s License, and good driving history.
  • Must be located in the Elizabeth, NJ area.
  • Must be willing to travel locally 80% of the time.
  • Must be licensed in NJ.

Nice To Haves

  • Current, unencumbered Registered Nurse license in NY (preferred, required within 60 days of hire date).
  • Experience in case management of workers comp cases.
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred.
  • Bilingual Spanish.

Responsibilities

  • Medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation.
  • Work with insurance carriers, medical care providers, attorneys, employers, and employees.
  • Closely monitor the progress of the injured worker.
  • Report results back to the employer and insurance carriers.
  • Willingness to learn and take catastrophic cases.
