About The Position

This position requires expertise in the nursing process and uses critical thinking skills to plan and coordinate patient care. This role collaborates with other team members to identify Medicare patients appropriate for Initial and Annual Wellness Visits (AWV) and execution of guided processes to complete the AWV. Assists with the triage of patient calls and completion of other clinical tasks as needed. Assesses patient healthcare needs that require additional follow-up by primary care provider and assists with scheduling appointments. Collaborates to facilitate completion of Maryland Primary Care Program goals. Provides knowledge of community resources to patients and connects them with resources. Serves as patient advocate and assists in identification and improvement of service delivery.

Requirements

  • Current Maryland RN license required.
  • Two (2) years of nursing experience in primary care, community nursing, critical care, or home health required.
  • Demonstrated skill in clinical case management.
  • Demonstrated skill in performing complete biopsychosocial assessments and medical triage.
  • Demonstrated skill in effective critical thinking.
  • Demonstrated skill in written and oral communications.
  • Demonstrated skill in age-appropriate interpersonal interactions.
  • Demonstrated knowledge of payer (incl. Medicare and Medicaid) regulations and programs.
  • Demonstrated knowledge of general internal medicine topics.
  • Demonstrated knowledge of case management principles.
  • Demonstrated ability to communicate and collaborate effectively with both internal and external customers.
  • Demonstrated ability to assess, adapt, and calmly respond to changing and/or crisis environment.
  • Demonstrated ability to make independent decisions consistent with current policies, procedures, and ethical standards.
  • Demonstrated ability to prioritize work assignments and manage time effectively to complete duties.
  • Demonstrated ability to develop a structured and organized process for monitoring active care plans.
  • Proficient computer skills, including Microsoft Office (specifically Word and Excel).

Nice To Haves

  • Completion of a Bachelor’s of Science degree in nursing preferred.
  • Strong knowledge of Epic EHR preferred.
  • Knowledge of population health principles preferred.

Benefits

  • Monday thru Friday 0800-1630
  • Pay Range: $42.64 - $64.00/hr. (depends on years of experience)
