About The Position

This role is in support of Cencora’s patient and provider support and pharma commercialization services in Canada marketed through our Innomar Strategies business. The Nurse Case Manager is responsible for the management of all aspects of program support for designated physicians or centers (hospitals or clinics) in a given geographic area, ensuring that patients receive easy access to the support program and their drug treatment. They also work directly with the reimbursement specialist to help obtain funding and support the pharmaceutical team as needed.

Requirements

  • A Diploma in health care or a related field, with a preference for Bachelor’s degree
  • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required.
  • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields
  • Ideal candidates should be nurses with healthcare and case management background in medical/hospital settings, insurance or bio-pharmaceutical industry
  • Fluency in French and/or English is required
  • Bachelor’s degree or diploma from a licensed nurse (RN, RPN/LPN), graduate studies or specific practice management experience is preferred, with five (5) years of nursing experience
  • At least two (2) years of experience in the context of public or private third-party reimbursement or in sales (not required, but an asset), case management or clinical support in the pharmaceutical industry, or related experience
  • The preferred candidate is a nurse with healthcare and case management experience in a medical office or hospital or in the insurance or biopharmaceutical industry
  • Bilingualism (French and English) is preferred
  • Experience in hematology and/or nephrology is an asset

Responsibilities

  • Provide support to patients and health care providers by offering information on the Patient Support services, specific diseases, and therapies based on manufacturer provided materials and product monograph
  • Assist patients and caregivers in conducting assessments related to Patient Support services
  • Act as a liaison between patients, families and health care providers related to medication access
  • Address general inquiries regarding program services, disease states, or product related queries based on provided materials
  • Offer continuous support on disease states or product-related questions to ensure patient adherence
  • Refer the patient to their healthcare providers for clinical questions beyond program materials
  • Document patient information and interactions within the electronic patient record
  • Conduct ongoing file review and updates to reflect patient journey
  • Follow case-management protocols, including data collection, patient follow-up, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers
  • Collaborate with the reimbursement specialist and the physician to secure funding for their patient’s therapies and explore alternative funding options
  • Ensure patient services are coordinated and tracked, according to KPI’s, resulting in expedited access to therapy
  • Participate in specialized training sessions for specific therapies provided by the pharmaceutical team
  • Attend pharmaceutical meetings, patient meetings, conferences, and work with patient associations and advocacy groups as needed
  • Coordinate with health care providers, pharmacies and infusion/injection clinics for treatment logistics and issue resolution
  • Maintain clear communication with the manager, team and the client regarding the management of patient cases
  • Assist in the training process of new Nurse Case Managers and provide feedback on protocols, databases, processes and inter-departmental processes
  • Report Adverse Events / Severe Adverse Events following program procedures
  • May be required to be on standby/ on call as part of this role
  • Provide medical support to patients and healthcare providers, including but not limited to, education on specific diseases and treatments and disease management
  • Provide telephone support (including inbound and outbound maintenance and adherence support calls) as needed
  • Database management and continuous verification of records and entered information
  • Administer and adhere to case management protocols, including clinical monitoring, data collection, continuous patient follow-up, compliance, monitoring of treatment interruption or discontinuation, adverse event reporting and liaison with primary healthcare providers
  • Prepare for and attend specialized advanced training sessions on specific therapies provided by the pharmaceutical team
  • Work directly with the reimbursement specialist and the physician to obtain funding for their patients' treatments; assist in finding other sources of funding as needed
  • Attend pharmaceutical sales meetings as needed
  • Attend patient meetings and conferences as needed to educate patients and providers regarding disease/product information and case management services
  • Work with patient associations and advocacy groups
  • Establish and maintain partnerships with the pharmaceutical team
  • Liaise with healthcare providers, pharmacies and infusion clinics and other organizations as needed to coordinate infusions, medication administration and resolve issues that arise
  • Clear communication with the manager, team and client regarding patient case management
  • With managers, participate in the training process of new nurse case managers, including but not limited to, observation and call monitoring
  • Support and collaborate with staff from other programs in the development and implementation of disease management programs, including establishing clinical requirements, writing texts, patient manuals or brochures, determining compliance program parameters, needs assessments and protocols
  • Provide feedback on current protocols, databases, processes and interdepartmental relationships
  • Be available on-call outside regular hours and on weekends (24/7) for periodic telephone consultations and emails as needed by the program
  • Be responsible for reporting adverse events to the units/departments assigned
  • Other functions and tasks may be assigned to the nurse case manager from time to time, as needed
  • May be required to work on-call or standby.
