About The Position

The Nurse Case Manager provides direct client care, assessing clients for changes in medical or nursing status under the supervision of a Medical Doctor. This role focuses on adults and adolescents with head trauma and other injuries, managing their medical/nursing needs until discharge. Responsibilities include observing and documenting client behavior, role-modeling, interacting with clients, training staff, ensuring client safety, and reinforcing treatment plans and organizational policies. The position acts as a liaison between various departments and provides case management services under the supervision of the Director of Tideway. The Case Manager facilitates staff and family meetings, advocates for clients, schedules activities and appointments, evaluates treatment programs, maintains contact with families and funding sources, and promotes rehabilitative progress. Flexibility and confidentiality are essential.

Requirements

  • Current Texas state license as a RN.
  • Knowledge of TBI and related syndromes.

  • Knowledge of counseling techniques and conflict resolution practices and the ability to implement them therapeutically.
  • Demonstrate good organizational skills.
  • Knowledge of the multiple and complex needs of the brain injured population.
  • Knowledge of, and ability to document according to facility standards.
  • Excellent communication skills.
  • Ability to work as a team member and facilitate the efforts of the treatment team.

  • Ability to cope with stressful situations.
  • Knowledge of licensing and accreditation standards (i.e., CARF, TRC, Medicare/Medicaid).
  • Ability to assist with transfers up to 150 pounds.
  • Ability to lift up to 100 pounds without assistance.
  • Graduate of an accredited school of nursing.
  • Hold a current license to practice as a Registered Nurse in the state of Texas.
  • Required CPR certification.
  • Required Texas driver's license.

Nice To Haves

  • Prefer 2 years post-graduate experience in nursing, preferably in the fields of rehabilitation, patients with cognitive deficits, or mental health issues.

Responsibilities

  • Perform specified nursing services as needed by clients.
  • Assess, monitor, intervene, document, and report clients' social and interpersonal behavior and functional ability.
  • Carry out therapeutic activities as designated by the Medical Director and Director of CQI.
  • Follow behavior modification program recommendations and submit documentation.
  • Assist in emergency situations, including fire alarm procedures and first aid.
  • Monitor client self-administration of medications.
  • Act as medical supervisor for clients on field trips, providing documentation of client responses.
  • Assist with specified therapies at the request of the Medical Director or Director of CQI.
  • Serve as a role model for clients and residential staff.
  • Attend in-services, workshops, staff meetings, and seminars.
  • Maintain confidentiality of client and staff information.
  • Provide a safe and secure environment for clients and staff.
  • Discharge duties in accordance with organizational policies, CARF, and other accrediting/licensing bodies.
  • Assist in the training of Residential and Tideway staff.
  • Perform INR/PT testing.
  • Maintain medical documentation, including medication monitoring sheets, incident reports, and medical consults.
  • Act as a therapeutic liaison between the Medical Director, Director of CQI, and Residential/Tideway staff.
  • Maintain security procedures.
  • Maintain appropriate records of work schedule on timecard.
  • Conduct pre-admission screenings with support from Business Development.
  • Review client's pre-admission documentation and information from screenings, sharing with relevant staff.
  • Conduct family interviews during the intake process.
  • Ensure reviews of client progress, case management notes, and other relevant documentation are forwarded to the program record.
  • Attend all staff and quarterly meetings concerning clients on caseload.
  • Perform advocacy services for the client during treatment plan implementation.
  • Discuss plans, goals, and status with the client and significant others to ensure client involvement and family education.
  • Provide a copy of the treatment plan to the family/guardian and funding source after obtaining releases.
  • Assist with scheduling medical appointments when the client or family cannot.
  • Explain rules, regulations, and rights of the individual being served regarding conflict resolution.
  • Assist the individual and family with travel arrangements for weekends, holidays, and emergencies.
  • Assist with monitoring medication problems in coordination with physician/facility nurse.
  • Coordinate and assist with the development of discharge plans.
  • Provide appropriate referral information for discharge community or alternate placement.
  • Work on-call as required and available after hours as needed.
