About The Position

Seniority Healthcare is seeking a compassionate healthcare professional to join their team as a COMPASS Nurse. COMPASS stands for Collaborative Outpatient Management for Palliative and Aging Support Services. This role involves providing in-home nursing and case management services to geriatric patients, with a focus on making a significant impact on their lives. This is a full-time position, Monday through Friday, with no weekend or on-call responsibilities.

Requirements

Minimum of 2 years of experience in a similar role, working with geriatric populations.
LPN or RN license.
Proficiency in EHR systems.
Strong communication skills and ability to work effectively in a team environment.
LPN or RN (Required)

Nice To Haves

In-depth knowledge of health care services and medical terminology preferred.

Responsibilities

Communicate effectively with patients, families, providers, and community staff regarding all aspects of patient care, including updates on treatment plans, concerns, or changes in condition.
Manage and coordinate patient care, including documenting and assisting with orders for testing, medications, and referrals.
Upload and document all health information into the patient's chart accurately and timely.
Obtain vital signs and assist providers as needed during patient visits.
Conduct medication reconciliation and maintain a professional relationship with the PCP.
Coordinate referrals, therapy orders, and telemedicine appointments.
Alert providers of acute issues.
Check for patient updates and communicate with case management departments.
Maintain HIPAA compliance and communicate effectively with other members of the care team.
