About The Position

The Case Manager serves as a member of the interdisciplinary team, assumes planning, responsibility, and accountability for resident care of a designated unit, in conjunction with the Unit Manager (UM) and/or Assistant Director of Nursing Services (ADNS), and in accordance with Federal and State regulations and company/facility policies, procedures and care plans.

Requirements

  • Current and valid Registered Nurse license in the state of Indiana.
  • Proficient medication management skills.
  • Ability to conduct thorough assessments and accurately document changes in resident conditions.
  • Strong passion for geriatric nursing and commitment to senior care excellence.
  • Demonstrates C.A.R.E. values to our residents, family members, customers and staff.
