About The Position

Our Nurses are supportive to those who are pretty vulnerable, and you will be a guide to our clients as they gain a better understanding of how mental illness impacts their lives. You will play a great support and coaching role that will help them improve their lives. Your work with probation is key, as it is seen for this population, their mental health is better served in treatment than in jail. This role is in an integrated team dedicated to deflect and redirect clients from incarceration and advance them to treatment. You will work within a behavioral health home clinic and get them stabilized in their communities. This role involves spending time directly with the clients providing outreach and education on meds, health coaching, supportive counseling and being a trusted knowledge resource of mental health recovery. We are looking for someone with a passion for advocacy for high-risk populations with substantial trauma history and who has challenged negotiating any systems. Requirements

  • Bachelors in Nursing and/or RN License in CO
  • Mental health experience
  • Collaborative and client-centered with experience in an integrated center
  • Organization to manage client information, appointments and follow up items
  • Collaborative team member
  • Passion for populations who are high risk and vulnerable
  • Experience with whole client care assessments and referrals
  • Experience coordinating with systems, including prescribers and referrals to medical and dental
  • Experience engaging clients in their whole health

Responsibilities
