About The Position

The Nurse Care Manager role focuses on building relationships with patients and coordinating care with various healthcare teams, including primary care, specialty care, and inpatient facilities. This position involves developing care plans, monitoring health information technology (HIT) tools to identify patient needs, and managing medication alerts, hospital admissions, and discharges. The role also includes monitoring and reporting on performance measures and outcomes.

Requirements

• Active Missouri RN license
• Ability to communicate clearly and effectively with co-workers and patients
• Ability to complete competency CPR and First Aid
• Ability to prioritize and compete multiple tasks simultaneously.
• Effective oral and written communication skills.

Nice To Haves

• Clinical experience preferred but not required
• PCHH and/or Chronic Care Management nurse experience

Responsibilities

• Builds relationships with patients and coordination with primary care team, specialty care teams, and inpatient facilities.
• Develops care plans for patients with Program.
• Monitors HIT tools and reports to identify gaps in care and needed services for patients within program.
• Addresses medication alerts, hospital admissions and discharges, including emergency visits.
• Monitor and report performance measures and outcomes.
