Job Description The Nurse Care Manager serves as a key clinical resource within our Behavioral Health program, ensuring patients receive coordinated, high-quality care across medical, behavioral health, and community settings. This role combines direct nursing care, care coordination, patient education, and clinical advocacy to support improved health outcomes and greater access to essential services.

Requirements

Associate's Degree in Nursing or equivalent.
Active Registered Nurse (RN) license issued by the Rhode Island Department of Health.
Experience providing nursing care in a clinical or community healthcare setting.
Experience coordinating care for patients with complex medical and/or behavioral health needs.
Valid driver's license with the ability to travel between sites and community locations as needed.
Exercise sound clinical judgment and make informed care decisions.
Build strong working relationships across disciplines and community partners.
Effectively manage multiple priorities in a fast-paced healthcare environment.
Communicate clearly and professionally with patients, families, and providers.
Analyze clinical information and respond proactively to patient needs.
Maintain accuracy, organization, and attention to detail.
Demonstrate accountability, professionalism, and confidentiality in all interactions.

Nice To Haves

Background in psychiatric or behavioral health nursing.
Familiarity with integrated care models and Behavioral Health Home programs.
Experience supporting diverse populations with chronic and behavioral health conditions.
Knowledge of PrEP navigation, gender-affirming care, and whole-person care approaches.

Responsibilities

Complete comprehensive patient assessments and identify barriers to care.
Create and maintain individualized care plans that address both medical and behavioral health needs.
Coordinate services with providers, hospitals, pharmacies, labs, specialists, and community agencies.
Monitor patient progress and adjust interventions to support improved clinical outcomes.
Facilitate access to healthcare services, social supports, transportation, housing resources, and other community-based assistance.
Follow up on hospital discharges, laboratory results, referrals, and ongoing treatment needs.
Conduct clinical triage for both walk-in and telephone patient encounters.
Administer medications, injections, and other prescribed treatments.
Monitor medication effectiveness and adverse reactions.
Assist with prior authorizations and medication-related documentation.
Support specialized clinical services, including toxicology screening and Clozaril management.
Maintain accurate, timely documentation within the electronic health record.
Educate patients on chronic disease management, behavioral health conditions, and medication adherence.
Promote patient engagement and self-management strategies.
Provide guidance and support to family members and caregivers when appropriate.
Partner with multidisciplinary teams to deliver integrated care.
Participate in case reviews, clinical meetings, and care planning discussions.
Support quality improvement initiatives, grant reporting, and outcome tracking.
Help strengthen community partnerships and referral networks.
Contribute to the ongoing development of clinical workflows and best practices.
