About The Position

The Nurse Care Manager is a Registered Professional Nurse who demonstrates expanded knowledge and experience in providing care for Health Home participants. The Nurse Care Manager represents and promotes the best interests of the Health Home program, care team, and patients cared for.

This is a full-time position, scheduled for 80 hours per two-week pay period, with typical hours of Monday through Friday, 8:00 a.m. to 4:30 p.m.

Requirements

• Graduate of an approved professional nursing program.
• Licensed to practice in the State of Missouri.
