About The Position

The Nurse Care Manager is an LPN who plays a crucial role in delivering patient-centered care by focusing on clinical assessments, care planning, and patient education. Working closely with field-based providers, the Nurse Care Manager assesses patient needs, coordinates care, and ensures that clinical care plans are effectively implemented to achieve positive health outcomes.

Requirements

  • Prior experience in a patient facing capacity required
  • Licensed Practical Nurse (LPN) with an active license; compact license preferred.
  • Excellent communication skills
  • Comfortable assessing status of patient remotely, active listener, motivational interviewing skills
  • Ability to recognize condition specific symptoms
  • Organized, thorough documentation skills
  • Solid time management skills
  • Ability to work independently as well as part of clinical care team
  • Ability to develop and build trusting relationships with patients and partners
  • Able to use Microsoft Excel for tracking and reporting

Nice To Haves

  • Associate degree preferred
  • Familiarity with Care Management preferred
  • Experience with Transitional Care Management strongly preferred
  • Minimum of 3-years of experience in geriatric or complex patient care
  • Experience working with Electronic Health Records (EHR) software and a Heath Information Exchange

Responsibilities

  • Conduct initial and ongoing clinical assessments of patients, inquiring about vital signs, gathering all information regarding pt condition or issue, prioritizing pt issues, assigning risk category and documenting all patient care data in the Electronic Health Record (EHR).
  • Contribute to the development and updating of individualized care plans in collaboration with care team and providers.
  • Maintain productivity metrics.
  • Coordinate with providers, family members, and caregivers to facilitate the implementation of care plans, ensuring all parties are informed and engaged in patient care.
  • Provide education to patients and caregivers on treatment plans, medication management, and other care needs to improve patient outcomes and adherence.
  • Handle medication-related inquiries, process prior authorizations, and document all interactions with patients regarding medication adjustments.
  • Review and process clinical orders as needed, ensuring compliance with provider instructions and patient needs.
  • Ensure all clinical documentation meets regulatory and compliance standards, adhering to HIPAA and company policies.
  • Participate in regular care management meetings and quality assurance checks to maintain high standards of patient care.
  • Work closely with Care Facilitators for scheduling support and non-clinical inquiries to focus on core clinical duties.
  • Participate in team meetings and provide insights on patient care processes, identifying areas for clinical improvement
