About The Position

We have an exciting Nurse Care Manager opportunity in our plant operations in Guymon, OK. The Registered Nurse Care Manager is an office-based medical and telephone medical care management position that is responsible for managing the Seaboard Foods medical management program with an emphasis on developing patients’ plans of care, providing education, guidance, and support for individuals with underlying health conditions for all employees at Seaboard Foods processing plant. This individual is responsible for early intervention and assessment, appropriate testing, medical care coordination, recordability review, and communication with appropriate government agencies and internal stakeholders.

Requirements

  • Bachelor’s degree in Nursing (BSN)
  • Current, unrestricted Registered Nurse (RN) license in the applicable state.
  • Minimum of three (3) years experience in direct clinical care to patients required.
  • Must be proficient in Microsoft Office suite with such programs as Word and Excel, as well as a comfort level with other software programs.
  • Strong clinical assessment, critical thinking, communication, leadership, and organizational skills.
  • Ability to proactively identify health concerns and develop appropriate preventive interventions.
  • Ability to work effectively with employees and leaders at all organizational levels.
  • Strong computer, documentation, and data-management skills.

Nice To Haves

  • Master’s degree in nursing with valid state licensure.
  • BLS/CPR certification.
  • Spanish speaking ability highly preferred.
  • Experience developing employee wellness, health promotion, and preventive health programs.
  • Case Management, Medical Management, Field Case Management, Public Health Care, or worker’s compensation-related experience is a plus.

Responsibilities

  • Manage a medical management system to assist employees with underlying health conditions.
  • Conduct initial and ongoing patient assessments, including medical history and record reviews, and develop individualized plans of care in collaboration with the employee and their primary medical provider. Act as liaison between the employee and medical providers to ensure all employees receive timely access to quality medical care while promoting optimal clinical outcomes.
  • Facilitate and monitor patient outreach and education to improve employees’ overall health and care while the medical treatment and virus management progress for all employees while working as an intermediary between medical care providers, management, and employees.
  • Proactively identify trends, conditions, and workplace factors that may contribute to employee health concerns and develop appropriate preventive health initiatives.
  • Promote early identification and intervention for potential medical issues to help minimize health-related disruptions and improve employee well-being.
  • Develop and implement employee health programs focused on wellness, health education, disease prevention, and maintaining a healthy workforce.
  • Monitors patient progress against the plan of care goals, emphasizing patient care needs during transitions and health changes.
  • Assist employees in explaining and reinforcing physicians’ instructions and answers to other questions the employees may have regarding treatment plans.
  • Educate employees regarding health promotion, disease prevention, ergonomics, appropriate use of medical services, and early reporting of health concerns.
  • Maintain accurate and confidential employee medical records in accordance with HIPAA and company requirements.
  • Partners with appropriate internal teams, when employee health concerns, intersect with broader workplace processes, while maintaining the medical department's clinical responsibilities and scope.
