About The Position

Brightli is seeking a dedicated Nurse Care Manager, RN to join their collaborative and mission-driven team. In this role, you will help individuals navigate complex medical conditions, promote overall wellness, support preventive care initiatives, and advocate for improved health outcomes. The Nurse Care Manager works closely with internal and external treatment team members to provide education, coordination, and support to individuals who have or are at risk for complex medical conditions. This position plays a key role in treatment planning, chronic disease management, health education, care transitions, and improving overall health outcomes through person-centered nursing care and collaboration.

Requirements

  • Active Registered Nurse (RN) license required.
  • Current driver's license, acceptable driving record, and current auto insurance required.
  • Excellent written and verbal communication skills.
  • Ability to implement innovative systems and processes.
  • Ability to make sound clinical decisions using critical thinking and nursing judgment.
  • Ability to work collaboratively with internal and external healthcare partners.
  • Successful completion of background check, including criminal record, driving record, abuse/neglect screening, and fingerprint check.

Nice To Haves

  • Experience working with individuals with Intellectual and Developmental Disabilities (IDD) preferred.
  • Knowledge of chronic disease processes affecting children and adults.
  • Strong understanding of nursing processes, care coordination, and patient-centered care.

Responsibilities

  • Develop and implement wellness, prevention, and health education initiatives for individuals with complex medical needs.
  • Facilitate health education groups and provide training on medical diagnoses, treatment plans, medications, and chronic health conditions.
  • Collaborate with clients and treatment team members to develop individualized healthcare goals and treatment plans.
  • Participate in the initial treatment planning process for Healthcare Home enrollees.
  • Consult with multidisciplinary treatment teams regarding identified health concerns and chronic disease management.
  • Coordinate hospital admissions, discharges, and transitions of care with medical providers and healthcare systems.
  • Monitor, track, and report performance measures and health outcomes using specialized nursing and healthcare information systems.
  • Implement and support required health technology programs and initiatives, including CyberAccess, Care Manager, and IntellectAbility.
  • Respond appropriately to emotional crises by accessing emergency services and coordinating rapid assistance when needed.
  • Participate in meetings, quality assurance activities, program evaluation efforts, and other assigned initiatives in accordance with organizational standards.
